Personal Data Processing Agreement

I, being the subject of personal data (hereinafter referred to as the «Subject»), freely, willingly and in my own interest, make the decision and express the consent to the processing LLC PLP Polifas INN 7840474619. (hereinafter referred to as the «Contractor») of my personal data received by the Contractor from me by filling out forms on the polifasplus.ru.

The processing of personal data in accordance with the Federal Law No.152-FZ of 27.07.2006 «On Personal Data» means the actions of LLC PLP Polifas (operations) with personal data, including collection, systematization, accumulation, storage, rectification (update, change), use, depersonalization, blocking, destruction of personal data.

The list of personal data for the processing of which consent is given: surname, name, patronymic, contact phone numbers, e-mail, information obtained when recording telephone conversations using the phone numbers used by the Contractor, the implementation of which does not pursue the purpose of collecting information about a specific person, but the purpose of using which is to control and improve the quality of the services provided by the Contractor, to ensure the safety of the Contractor, visitors and third parties, but if illegal actions are detected, they can serve as evidence of these actions.

The period for processing personal data is the period of existence of the Contractor. Before the expiration of the period for processing personal data, the Contractor has the right to automatically depersonalize the personal data of the Subject, namely, actions as a result of which it is impossible to determine the belonging of personal data to a specific subject of personal data. At the same time, the data is saved to support the business processes of the Contractor, including for analysis and statistics of activities.

Personal data is stored in the information system of the Contractor until the termination of the Contractor’s activities, while it can be erased by the Contractor at any time without notifying the Subject.

The purpose of personal data processing is to identify persons when the Contractor provides the Services, records the Services provided, transfers personal data to other contractors (including, but not limited to: sending and / or communicating to the Subject information about the Contractor’s activities, promotions held by them or with their participation sent by the Contractor).

In the manner prescribed by the current legislation of the Russian Federation, the consent can be revoked by the Subject by written appeal to the operator who receives the consent of the subject of personal data.

I agree that, upon my written request, a notification of the destruction of personal data will be delivered to me (my representative) at the location of the Contractor.

I agree to receive, through voice, SMS messages, messages of other information by e-mail, by phone specified above or provided to the Contractor and / or a person authorized by him in another way, the following information sent by the Contractor or on behalf of third parties: information about Services and other services, payments, data on payment arrears, changes in the rules and procedure for the provision of services and other changes in the activities of the Contractor, promotions held by him or with his participation.
